I am wondering what the TCM definition of “phlegm” is. What is extreme heart fire, extreme phlegm fire and fire harassing the heart? And I guess it would be the shen.

Phlegm is the same as western medicine defines it - thick mucus. Putting extreme in front of a condition is likely either arbitrary or the method a particular practitioner uses to denote the severity of the condition. From a textbook perspective there is no separate diagnosis of extreme anything. I’ve never come across the term ‘extreme phlegm fire’ before. We have a page on heart fire.
